To move away from the Japanese culture perspective for a moment, I was just trying to place exactly what it is about lactating women that is so appealing. And I’d say it’s one or more of the following:
1) Fertility symbol. A woman lactating (especially the ones which really gush and drip) sets up that she is a good candidate for nursing potential offspring, which “pulls some strings” subconsciously in the back brain forcing a strong, primal urge to breed.
2) Visually appealing. Building on from the very long established “big tits = fertile” thing pop culture is aware of and point one, there is something about the fullness and movement of a milk filled pair of tits on a woman that brings on a certain warm, fuzzy feeling.
3) Sex and food. As in many cases it is possible to combine intercourse with feeding in the case of two or more consenting adults, it could be said that a lactating woman is fulfilling two needs: that of sexual release/intimacy and sustenance, with the latter fueling the former. Like an erotic ouroboros, if you will.
4) Comfort/infantalism. Now the latter word is often a taboo even among fetishists, but let us remember there are degrees of this: it’s not adult baby or nothing. Personally I find the adult baby think unsettling and baffling, but I have an idea why it’s a fetish. Again, subconscious string pulling and the need for intimacy meaning this occupies a similar position to point three. In the case of a hetero relationship, the woman can give all kinds of support/care to her man, and being able to taste of her is just an extension of this.
5) Health reasons. Breast milk is medically proven to be an excellent source of nutrition for adults and newborns alike. That’s appealing on a couple of different levels.

As for the nurse thing, the appeal doesn’t lie always in the uniforms, but more the ideals it represents: care/nurture/comfort and the twisted taboo of using them in a more aggressively sexual context. Combine and re-enforce this with the lactation points I mentioned and it should be easy to spot why a milkmaiding nurse could be considered extremely erotic by certain types of people, although personally I’d take mine less submissive, but then I like strong women or at least ones that are equal in that regard.
